chronic back pain is one of the most common presenting complaints that general physicians encounter. The vast majority of these are not related to a malignancy.
Yes, there are many types of cancer that could cause chronic, unrelenting back pain - but not for years. If there is a malignancy in the spine, it will usually threaten life within a year. Multiple Myeloma in the spine and any carcinoma or sarcoma metastatic to the spinal column could cause unrelenting pain and chronic fatigue, but many other benign conditions that are more common could cause these symptoms.
This must be evaluated by a physician who knows all the details of your medical history and physical examination plus blood work and x-ray studies.

I bet you have two different things going on, not related except if you are taking some kind of medication for your pain that is causing the exhaustion.
The fatigue might be that your body is fighting the pain so much it wears it out.
Get a blood test done to see if you are anemic, that is an easy fix.
I work in a hospital and have seen one case in 15 years be cancer from just back pain.
Something tells me you might need to take vitamins big time…and drink more water than you are now.

The history of cancer in the family is that old "power of suggestion" at work again. Instead of cancer why not think arthritis maybe? Arthritis is no fun either (I have it bad) but it’s a better diagnosis than cancer (I have that too). You need to see your doc and have a chat. Best wishes to you for a long and cancer-free life!
